Brookings Chiropractic Center, a Telligen Quality Payment Program (QPP) Resource Center client in Brookings, SD, recently achieved the National Committee for Quality Assurance’s (NCQA) Patient-Centered Connected Care Recognition.

This recognition is awarded to practices that provide outpatient care, but do not act as the patient’s primary care provider. The goal is to close gaps in care by connecting relevant patient information back to the patients’ primary care team in a structured and timely manner.

Achieving this title requires clinicians meet an aggregated 80 percent score on specific clinical measures, which exceeds the quality benchmarks for some those measures. In addition to this three-year recognition, the group will also receive credit for a medium-weighted Clinical Practice Improvement Activity in 2018.

What is the NCQA Patient‐Centered Connected Care Recognition Program? 

According to the NCQA, the program, “will evaluate sites delivering outpatient health care  that communicate effectively with a patient’s other providers—especially primary care  providers that constitute a patient’s medical home. The result is integrated, patient‐centered care for patients receiving care at multiple different care delivery sites. The recognition uses evidence‐based standards to evaluate how care teams collaborate with patients and how a care site connects to other providers.”

What Are The Program Requirements?

Requirements include activities many clinicians already do, but might need to improve documentation. Practices must meet a minimum score across five different standards including:

  • Connecting With Primary Care: Helping patients connect with a primary care provider and sharing information with that provider
  • Identifying Patient Needs: Directing patients to appropriate providers, when necessary.
  • Patient Care and Support: Using evidence-based decision support in care delivery, collaborates with patients to make care decisions and delivers culturally and linguistically appropriate services.
  • System Capabilities: Using electronic systems to collect data and execute specific tasks.
  • Measure and Improve Performance: Systematically monitor performance and carry out activities to improve clinical outcomes and patient experience.
