This AHRQ toolkit provides resources, tools, guides, and educational materials to improve the hospital discharge process. Research has shown that this Re-Engineered Discharge (RED) is effective at reducing readmissions and posthospital emergency department visits, so it has been developed into a toolkit to help hospitals replicate it.
