FULL SERVICE POPULATION HEALTH SOLUTIONS

Disease Management Coaching

This program is available for members who have been diagnosed with a chronic condition. Telligen’s certified coaches help members to understand their conditions. We coordinate healthcare treatments and collaborate with providers on evidence-based practices to improve the health of members and manage costs.

At-Risk Coaching

Telligen’s Metabolic Syndrome or At-Risk Coaching Solutions are available to members who have elevated blood pressure, weight, cholesterol, triglycerides, and/or blood glucose. Telligen health coaches engage with members to help educate them to understand and manage their risk factors for healthier outcomes.

Case Management

Following a health crisis or a new diagnosis, Telligen’s nurse case managers engage members in a collaborative process of health assessments, planning, care coordination, education, and advocacy of care. This is all done with the final goal of members having the ability to successfully selfmanage their care and condition.

Utilization Management

As required by the City’s healthcare plan, the goal of our Utilization Management (UM) program is to provide a fair, evidence-based review of the care you are receiving to determine medical necessity. The UM program protects you from receiving treatments that do not meet the standard of care.

Targeted Health Programs

With a commitment to delivering top-tier health solutions, Telligen ensures a personalized approach for each client, crafting interventions and strategies that align with the unique characteristics of their population. This dedication to customization enables Telligen to offer not only full-service population health management but also targeted programs designed to meet the specific health needs of diverse client populations.

Testimonials

“This program made me more committed to health changes. I have tried other methods before with some success but was never really committed to my health like I am now. The Telligen method give you knowledge of your health issues and suggests ideas on how to improve your well-being.”

“The program is easy to participate in. The phone calls are scheduled at a convenient day and time. The advice given is excellent, and I can talk to my coach like an old friend. Being able to talk to someone with knowledge and an open view is unmeasurable. I have had positive progress thanks to the program.”

I consider the contact and relationship established an integral & essential aspect of my treatment and recovery. Having a  person who is outside of your actual care team monitoring and charting your progression is a vital element often missing in today’s medical treatment systems.”
