Client & Program: SoonerCare Health Management Program, Oklahoma Health Care Authority
Description: Since February 2008, we have partnered with the Oklahoma Health Care Authority (OHCA) to develop, implement and operate the SoonerCare Health Management Program (HMP).
The program is a statewide initiative designed to improve the clinical outcomes and lower the cost of care for Medicaid members who have or are at high risk of developing chronic disease through Practice Facilitation, Health Coaching and Resource Management.
Result:
- Combined lifetime program savings of $364.3 million
- Produce an average ROI of $2.90 in state savings for every $1 in expenses over the past six years
- 90 percent of providers credit the program with improving management of patients with chronic conditions
